Clinic "Sick Bay"

Complete Emergency Contact Form in FOCUS portal and submit directly to school nurse.

 

SICK BAY VISIT POLICY:

 

Only CLINIC PASSES from a teacher will be accepted in order for student to enter the clinic.
 
If there is NO PARENTAL CONSENT for health services, student will be returned to class or parent MUST PICK STUDENT UP or arrange for pick up within 30 minutes.
 
The only exception to this policy is in an emergency situation deemed so by RN and clinic staff. 

 

MEDICATION INFORMATION:

 

New Medication Forms must be completed EVERY SCHOOL YEAR and if any changes to the current order by provider.
If your student has any of the following conditions, forms are attached below to be filled out by their medical provider. These forms must be returned to the clinic by a parent/guardian in addition to any prescribed medications. 
Diabetes - Form to be obtained from student's Endocrinology Provider.
