Clinic "Sick Bay"

Reminder that ALL students must complete an Emergency Contact Form AND a Student Directory Information Form in Focus.

Procedures to visit sickbay:

  • In non-emergency situations, a pass is necessary. A teacher will make out a pass to the sickbay (clinic) to be given to the clinic assistant.
  • If a student feels sick at any time, she/he should ask for permission to go to the sickbay (clinic). Then Castle out to the clinic or come with a pass from a teacher.

  • A Medication Administration Form 398/401 must accompany all medications to be given during the school day, including over-the-counter drugs.
  • With proper physician and parent permission, the student may carry their inhaler or epi-pen.
